A flashback introduces their father, Dr. Vetrimaaran, a compassionate village doctor who dreamed of affordable healthcare for all. His ideals threaten powerful hospital owner Daniel Arockiaraj, who has Vetrimaaran murdered and attempts to eliminate his family. The twins are separated in the attack — one raised in India as the magician Maaran, the other adopted abroad as the doctor Vetri.

Years later, Maaran begins eliminating those responsible for his father's death while exposing corruption in the medical industry. Vetri eventually learns the truth about his family, and the brothers unite to bring Daniel and his network to justice, fulfilling their father's dream of ethical, affordable healthcare.
